Privacy Policy — CK9 Rekam

1. Overview

CK9 Rekam ("the Extension") is a Chrome browser extension that records health screening form responses and automatically re-fills them on subsequent visits. This Privacy Policy explains what data the Extension collects, how it is used, and how it is stored.

2. Data We Collect

The Extension collects and stores the following data locally on your device only:

• Form field values — answers you manually fill in health screening forms and choose to record (e.g., radio button selections, numeric inputs, dropdown selections). These are stored in Chrome's local storage on your device.
• Form title — the title of the form page, used as an identifier to match the correct recording on future visits.
• Extension settings — whether auto-fill is enabled or disabled.

The Extension does not collect:

• Personal identity information (name, date of birth, ID number, address, etc.)
• Browsing history or data from any other website
• Medical records or patient data
• Passwords or authentication credentials

3. How Data Is Used

All recorded data is used solely to pre-fill form fields when you revisit a previously recorded form page. This reduces repetitive data entry for health workers and clinic staff.

4. Data Storage and Sharing

• All data is stored locally using Chrome's chrome.storage.local API.
• No data is transmitted to any external server, third party, or remote service.
• No data is shared with any person or organization.
• Data can be deleted at any time using the "Hapus Semua" button in the Extension popup, or by uninstalling the Extension.

5. Permissions Justification

• storage — Used to save recorded form values and extension settings locally on your device via chrome.storage.local.
• activeTab — Used to inject the recording and auto-fill script into the currently active tab when visiting the supported health screening portal.
• tabs — Used to query the currently active tab's ID so the Extension popup can communicate with the content script on the active page (to start/stop recording). No browsing history or data from other tabs is accessed or stored.

The Extension's content scripts are restricted exclusively to pages matching *://form.kemkes.go.id/*. The Extension has no access to any other website.

6. Children's Privacy

This Extension is designed for use by healthcare workers and is not directed at children. It does not knowingly collect information from children under 13 years of age.

7. Changes to This Policy

If this Privacy Policy is updated, the new version will be made available at the same URL. Continued use of the Extension after any changes constitutes acceptance of the updated policy.
